Take the Next Steps

As you begin your journey toward bariatric surgery, it’s important that you’re well-informed about both the causes of obesity and the surgical procedures we use to treat it. Understanding your options will empower you to make confident, educated decisions.

1. Watch the Online Seminar

To get started, please watch Dr. Mailapur’s online weight loss surgery seminar. This seminar covers:

  • What causes obesity
  • How bariatric surgery works
  • The differences between procedures
  • Who qualifies for surgery

Watching the seminar will prepare you for your initial consultation and help you identify which procedure may be best suited to your needs.

2. Complete your new patient paperwork

After watching the seminar, log in to your patient portal (Check your email for instructions) and complete the Seminar Acknowledgement and Health Questionnaire forms.

If you don’t have a portal log in, please contact our office.

Both forms must be completed in the Portal prior to your Initial Consultation with Dr. Mailapur.

3. Schedule an appointment

If you haven't already, please schedule a consultation with Dr. Mailapur. Madison Surgical Associates does not accept Medicaid. A $100 consultation fee is due at the time of your appointment.
