GLP-1 Medication Vs. Surgery

Understanding Your Weight Loss Options: GLP-1 Medications vs. Bariatric Surgery

GLP-1 medications and bariatric surgery both treat obesity, but they work differently and produce different results. The research increasingly favors surgery for patients with significant weight to lose. Which one fits your situation depends on where you are right now.

If you’ve been paying attention to health news over the past few years, you’ve heard about Ozempic, Wegovy, and the wave of GLP-1 medications that have changed how people think about weight loss. A lot of patients come to us having already tried one of these drugs, or wondering whether they should before considering surgery. It’s a reasonable question, and it deserves a direct answer.

We offer both. We prescribe GLP-1 medications under physician supervision, and we perform sleeve gastrectomy and gastric bypass. That means we don’t have a financial reason to push you one direction. What we have is a view of both treatments from inside a practice that uses them — patients who respond well to medication, patients who plateau and need more, and patients whose disease burden requires surgery from the start.

The research has gotten significantly clearer over the past year. A 2025 JAMA Surgery study of 30,000-plus patients found surgery produced nearly three times more weight loss than GLP-1 therapy over two years, at lower ongoing cost. A 2026 systematic review of 430,000 patients presented at the ASMBS Annual Meeting found surgery outperformed GLP-1 medications across every outcome measured — weight loss, diabetes remission, hypertension, and cholesterol improvement.

That doesn’t make medication the wrong answer for every patient. GLP-1 medications are a real treatment, especially for patients with cardiovascular disease, more modest weight loss goals, or a preference for starting without surgery. But for patients with a BMI over 40 or significant obesity-related health conditions, the evidence now points clearly in one direction.

“I never tell patients that medication is wrong,” says Dr. Mailapur. “But for someone who needs to lose 100-plus pounds, we have to be realistic about what each tool can accomplish.”

If you’re trying to work through this decision, we’ve put together a full breakdown of how the two treatments compare across weight loss, disease remission, cost, and risk — with the actual study data behind each claim. Read the full comparison here.
